I read in your letter column in which a Cuban doctor left a surgical instrument in a patient’s abdomen.
It is due to gross negligence not only by the doctor, but also by the nurse who was responsible for an accurate instrument count.
The doctor knows that he DOES NOT close the cavity until all the instruments and “swabs” (cotton wool, gauze) used during the surgery are accounted for.
At the beginning of any surgery that involves a cavity, a meticulous swab and instrument count is performed and recorded.
On completion of the surgery every swab, every instrument that was counted before the surgery should be lying around somewhere outside the patient’s body.
We all make individual human errors, but it is inexcusable when two are involved.
