There is going to be a scathing criticism of a public institution over a death. As has become the norm, there are going to be accusations of neglect and poor medical procedures.
One of the charges is going to be correct because the people who should have been performing the medical procedures would find other things to do elsewhere.
The medical institution is going to rush to the defence of the failed medical practitioners and blame the messengers for divulging the issue. But in the final analysis there is going to be an investigation and sanctions would be recommended.

Disaster is going to strike at a construction site. Workers there are not known to observe all the safety precautions. There would be a slip that would lead to serious consequences.

The roads are not going to get any safer because of the influx of new drivers who know nothing about road safety.
A major collision would not only leave a mass of twisted metal but would also leave a lot of blood and broken bones at the scene.
